Atypical presentation of Infantile hypertrophic pyloric stenosis, where a two month old child presented with non bilious vomiting since 1 week of life. Child had severe dehydration and visible gastric peristalsis at presentation. Upper GI contrast confirmed partial obstruction at the level of pylorus, USG abdomen also confirmed the diagnosis of IHPS. Intraoperatively thickened and fibrotic pylorus was found and pyloromyotomy was done.
